*$5 supply fee in addition to workshop fee
NOTE: A $10 nonrefundable deposit is due within one week after registration for each 5
week workshop. This deposit is a part of the workshop fee. It is not in addition to the fee.
The remaining balance must be received 7 days prior to the start of the workshop to
ensure a space for your student. Session Fees are nonrefundable after workshops
begin. For all one day workshops we ask that the full registration fee be sent 10 days
prior to the start of the workshop.

Liability Information

In the event of an emergency, I hereby authorize an instructor of these workshops to
secure appropriate medical care for my child(ren) listed above until a parent or
guardian is present in person. I also hereby release KnowHow Workshops and any
instructors of the workshops taken from all legal claims and liabilities which arise from
participation in these workshops.

Cost of each workshop: $55.00* per student per 5 week session workshop
*Students who take 2 or more workshops within the same session will receive a $5
discount per workshop. One Day Workshops are $30.00 (when offered)
2010 Spring Session workshops are for ages 10-14.
